Two familiar disasters look similar in the pan and come from opposite mechanisms. One is sugar crystallising when it should not, the other is fat and water refusing to mix.

Caramel grains when sugar comes out of solution

A boiling sugar syrup is holding far more sugar than water can normally dissolve, and that state is unstable.

Any seed, whether a crystal on the pan wall, a stray grain or an agitation, gives the sugar somewhere to start rebuilding its crystal structure.

Once started it propagates rapidly through the whole pan, turning a clear syrup into a grainy opaque mass in seconds.

Preventing it means removing the seeds

Washing sugar down from the pan sides with a wet brush dissolves the crystals that would otherwise fall in.

Covering the pan early lets condensing steam do the same job, which is why many recipes call for a few minutes with the lid on.

Glucose syrup or a little acid interferes with crystal growth chemically, and stirring is avoided entirely once the syrup is boiling.

Chocolate seizes when a trace of water arrives

Melted chocolate is cocoa solids and sugar suspended in fat, with almost no water present at all.

A small amount of water dissolves the sugar at the points it touches and creates a syrup that binds the dry particles into clumps.

The whole mass stiffens into a dull, grainy paste, which is why a drop of condensation or a wet spoon ruins a bowl of chocolate.

The fix for seizing is more water, not less

Adding enough liquid changes the mixture from dry particles bound by a trace of syrup into a genuine emulsion, which is smooth again.

This is why ganache works: sufficient cream is present from the start for the fat and water to form a stable dispersion.

Seized chocolate cannot be returned to its original state, but it can be turned into a sauce or a ganache and used that way.

Overheating is a separate failure

Chocolate held too hot separates, with cocoa butter pooling on the surface and the solids becoming grainy and scorched.

The proteins and sugars in milk chocolate burn at lower temperatures than dark, which is why milk chocolate is melted more gently.

Burnt chocolate is unrecoverable, unlike seized chocolate, since the change is chemical rather than a matter of arrangement.

No, since it is a structural fault rather than a doneness one, and the loaf is usually baked correctly. Check the internal temperature separately if you are unsure.

Can I still use a loaf with a big cave in it?

Yes, though the slices with the void in them are awkward for sandwiches. Toasting and cutting into smaller pieces makes it a non-issue.
