Underproved: dense but with height
An underproved loaf usually has a tight, uniform crumb with small even holes and a distinct dome, because the structure was strong but had too little gas in it. The score often opens dramatically and the loaf springs well, since the gluten still had plenty of stretch left to give. A dense ring of tighter crumb near the base is common, where the weight of the loaf compressed the least-developed part.
The fix is straightforward: extend bulk fermentation, raise the dough temperature, or both, and use a jar to measure the rise rather than a clock. This is the most encouraging of the five faults, because everything except the timing was already working.
Overproved: dense and flat
An overproved loaf spreads outward rather than upward, with a flat profile, a score that barely opened and a pale, blistered surface. The crumb is often irregular with a few very large holes near the top and a compressed, sometimes gummy layer at the bottom.
At oven temperature, this happens because the gluten network has stretched past its useful limit and can no longer hold the gas it made. The smell is a useful confirmation, since an overproved dough is noticeably alcoholic and sharply sour rather than pleasantly yeasty. Shorten bulk, shorten the final prove, or drop the dough temperature, and remember that warmer dough gives you a narrower window.
Weak gluten development
A dough that never developed enough gluten produces a crumb that is dense but coarse and irregular, with a slightly crumbly rather than chewy texture. The loaf often looks reasonably risen but feels heavy for its size, and slices tear rather than cutting cleanly. Common causes are a low protein flour used for bread, a very short mix with no folds, or too much wholemeal for the development the dough received.
At oven temperature, add two or three sets of stretch and fold during bulk, or switch to a flour with twelve per cent protein or more. A windowpane check before shaping confirms whether this is the problem or something later in the process.
Insufficient hydration
A dough mixed too dry never becomes extensible enough to hold large gas cells, so the crumb comes out close and tight regardless of fermentation. This happens most often when a recipe is followed by volume rather than weight, or when wholemeal flour is used at a white flour hydration. Wholemeal absorbs noticeably more water, often five to ten per cent more, because bran and germ hold liquid that white flour does not.
By the second rise, the dough feels tight and resistant during folds and the surface stays matte rather than becoming smooth and slightly glossy.

Shaping and degassing
A loaf can be perfectly fermented and well developed and still bake dense if shaping knocked out the gas or failed to build surface tension. Pressing the dough flat during shaping destroys the fine cell structure that bulk fermentation spent three hours building.
Once the crumb sets, a slack, untensioned surface lets gas escape sideways during the oven spring instead of pushing the loaf upward. Shape firmly but without crushing, aiming for a taut skin rather than a compacted mass, and give a preshape and bench rest. If bulk and prove look correct on paper but the loaf stays heavy, this is usually where the problem is.
Flour absorbs water differently by brand and by season; hydration is a target, not a rule.

Photograph the cut crumb every time, since a record of what different failures look like is worth more than any written description. Change one variable per bake and keep everything else, including flour, hydration and dough temperature, exactly the same.
Note the final dough temperature and the actual bulk duration, because those two numbers explain most fermentation faults. Most dense loaves in home kitchens are underproved rather than overproved, because home bakers work in cooler kitchens than recipes assume. If in doubt, give the dough more time at a measured temperature before changing anything about the ingredients.
